GERALD ALLEN CHOUINARD

      November 23, 1932 – June 5, 2025 (Age 92)

Gerald (Jerry) A. Chouinard passed away peacefully at his home on June 5, 2025. He was the youngest child of Archie and Marie (Schlumpberger) Chouinard, born in Minneapolis, Minnesota. After completing high school, he became a printer’s apprentice in Batavia Illinois. Jerry enlisted in the U. S. Army in February 1953 and was trained as a pole lineman, achieving the rank of Corporal with the 40th Signal Battalion and included foreign service in Germany.

In 1967 he founded The Label Printers in Chicago and grew the company from a one-man operation to an industry leader in packaging, labeling and product authentication. He invented and received a U.S. Patent for Counterfeit Resistant Labels in 1989 and enjoyed great success with many international customers, including Underwriters Laboratories.

Jerry semi-retired and moved to Hereford Az in 1991 where he co-founded with his wife Pat, Our Lady of the Sierras Foundation, a Marian Shrine. A 75-foot Celtic cross, statute of the Blessed Virgin Mary and chapel were constructed. The Foundation has now expanded to included a prayer house and La Purisima Retreat Center. The Shrine has welcomed pilgrims from all over the world who come seeking the peace and love of God.

He was preceded in death by his first wife Marlene Ann (Buhmann) Chouinard, his second wife Patricia (Tishman) Chouinard, brother Donald Chouinard, sister June Lodge, son Steven Chouinard, step-son Robert Callahan, step-daughter Donna Chrysokas.

Jerry is survived by his daughters Nannette Chouinard, Hereford Az, Suzanne Scardemaglia, St Charles, Il, son Gregory (Cathy) Chouinard, Hereford, Az, step-daughter Cheryl Callahan, Hereford, Az and step-son Steven (Anne) Callahan, Queen Creek, Az.

A Mass of the Resurrection will be held at St. Andrew the Apostle Catholic Church 800 Taylor Dr., Sierra Vista, Az on Saturday June 28, 2025 at 10:00 am, The Most Reverend Gerald F. Kicanas, presiding. Internment will follow the Mass at Our Lady of the Sierras, 10310 S. Twin Oaks Rd, Hereford and a celebration of life will follow at Mary’s Knoll Prayer House, 10235 S. Twin Oaks Rd, Hereford, Az.
